We had a similar experience with our latest litter, Paul, but at a slightly later age than yours. One of the bitches was very small, and very lightly framed. She was actually being bossed by the bigger pups and not getting her full share of the food. We split them up and now she is thriving. However, she is still going to be smaller than the others when she is fully grown.

The small pup needs a course of antiobiotics imo.
You should see immediate improvement.
It is very often a low grade infection of some kind and the sooner you start him on it the quicker he will catch up on his litter mates.

Sounds like he might just be going to be a small dog, Paul. Our bitch pup is a lot smaller than her sisters, but is perfectly healthy.
